Form: Initial Report

This form is used when presenting the report of findings for a new person.

The Initial Report form contains a summary of the presenting condition, physical findings and results of exams and tests for the new person. It also contains the recommendations for duration and frequency of initial care and the agreed action steps to be taken. This is placed in The Journey folder at the Report of Findings visit for the person.

The Journey Forms are a simple reference for your practice members as they make their transition towards a healthier life. These forms, in an easy to understand way, provide a consistent and congruent message.  All forms are sold in pads of 100.
